Prior to your return from leave, you will need to complete the “Fitness for Duty/Return to Work Release” in the Workforce Time & Attendance system. This document will be sent by the leave administrator upon approval of your leave of absence. If your FMLA leave was for your own serious medical condition, your Healthcare Provider will need to complete Section III.
