How to Request a Duplicate Form W-2 or W-2C
Form W–2 Wage and Tax Statements are printed and mailed to the address on record for all active and inactive employees by Jan. 31 of each year. Some employees may also receive a Form W-2C if any corrections were made to their Form W-2. SFUSD inactive employees can request a duplicate print Form W-2 in certain circumstances. Active employees can access an electronic copy of their W-2 online. W-2 records are available and provided by the district for up to four (4) tax years. |

Inactive employees who have lost, misplaced, or changed their address at the time of delivery of the W-2 or W-2C can request a reprint and pick it up in person at 135 Van Ness Ave. Please allow seven (7) working days for processing. A valid government-issued ID is required to pick up the document. |

Inactive employees no longer have access to EMPowerSF. Inactive employees who are unable to pick up a document in person at 135 Van Ness Ave. must request an unofficial, electronic version of their W-2 or W-2C online. Proof of a valid government-issued ID is required. |
Historical W-2s prior to 2022: Electronic only |
Employers must keep a record of historical W-2s for 4 years. Active employees can visit stubs.sfusd.eduto view W-2s from 2019-2021. |
